IMMUNIZATIONS Your child may need their immunizations updated before entering the new school year. Please refer to the 2021-2022 Ohio Immunization Summary for School Atten- dance which can be found on the District website or at bit.ly/3CnyALL . Children entering into kindergarten, sev- enth and 12th grades have required immu- nizations. Other students not following the recommended immunization schedules may also need immunizations to enter the new school year. The State of Ohio requires that all students entering school show proof of immunization. If proof of immuni- zation is not provided by the third week of school, the school may deny attendance to the student until proof is provided. Immunizations should be provided by your child’s health care provider. If your child does not have a health care provider in the area and immunizations are needed, local county community health depart- ments may be able to help. Greene County 5 Rivers Medical Center: 937-708-3400 Medical services provided to their patients; no immunization clinic; services (including immunizations) provided on a sliding scale for uninsured; accepts Med- icaid (except Molina); other insurance providers. Clark County: 937-390-5600 Immunization Clinic by appointment only; accepts some insurance and may have a minimal office fee. Call for more information. Montgomery County: 937-225-4550 Immunization Clinic by appointment only. Call for more information. Some local pharmacies and Minute Clinics offer some immunizations as well. Contact your preferred pharmacy for more information. MEDICATION REQUIRED AT SCHOOL Only FDA-approved medication pre- scribed by a healthcare provider and over-the-counter (OTC) medication can be given at school. Any medication must be authorized by a parent or legal guardian. All medication must be located in the school office/school nurse’s office, Yellow Springs Schools policy states that without excep- tion, students are not permitted to carry medication on their person or personal belongings unless necessary for an emer- gency situation (i.e., Epi-Pen or emergency inhalers). In order to carry emergency medi- cation an authorization signed by a parent and prescribing healthcare provider must be completed and on file for the current school year. All medication forms are located in the parent section of the website under “stu- dent medical forms” and must be com- pleted and on file for the student to receive medication at school. Any medication required on school- sponsored overnight field trips that are given on a regular basis at home must have signed (physician/parent) medication forms prior to the field trip. This medication must be brought to the school prior to the day of the field trip as designated by the spon- sor of this field trip. Any student requiring injections or special care for a chronic ill- ness must make arrangements for care in advance with the school nurse. CHRONIC ILLNESS AT SCHOOL We want our children with chronic ill- nesses to be successful by attending school as much as possible. A Licensed School Nurse (LSN) is available in each building to help manage this care while your child is at school. Please communicate these medical issues with the school so you and the nurse can collaborate and create an acceptable plan of care. VISION/HEARING SCREENINGS During typical school years, a vision and hearing screening is conducted on all stu- dents in grades K, 1, 3, 5,7,9, and 11, first- time Yellow Springs Schools students, any student initially receiving educational evalu- ations, and any student identified as having possible vision or hearing issues. Parents will be notified of any abnormal findings. With COVID-19 mitigation in the schools, these mass screenings have proven to be difficult. Our school nurses will conduct these screenings if it is possible to maintain safety for our staff and students. | FOOD SECURITY FOOD ASSISTANCE Yellow Springs Community Food Pantry Hours: Second and fourth Thursdays of each month, 2–4 p.m., at United Method- ist Church. Contact: 937-510-6030. Little Free Pantry Outdoor, free-standing pantry behind Presbyterian Church, across from Mills Lawn on Walnut Street. Dayton Street Pantry Outdoor, free-standing pantry on Dayton Street in Bill Duncan Park. Mobile Farmers Market Sponsored by the Foodbank of Dayton, this mobile food bank stops in the park- ing lot of First Baptist Church on the third Thursday of each month at 1 p.m. No proof of residency is required. TO DONATE FOOD YS PORCH provides a monthly food pick-up on porches in support of the YS Community Food Pantry. Contact yel lowsprings@porchcommunities.org in advance for their First Monday of the Month porch pick-up. Donations may also be made in person at any time to the free-standing pantries on Walnut and Dayton streets, and to the YS Community Food Pantry on Tues- day and Wednesday, from 9–11:30 a.m. Donated items that would be most helpful at all pantry locations are: canned meat (chicken or turkey), canned fish (tuna or salmon), peanut butter, hearty soup, pasta, canned pasta sauce, meals in a box (Hamburger Helper, Rice-a-Roni, macaroni and cheese, Pasta Sides), rice, canned beef stew and baked beans. Food with expired “use by” dates or damaged packaging cannot be accepted. FREE AND REDUCED LUNCHES The U.S. Department of Agriculture has extended its provision of free meals for students for the entire 2021–22 school year. This was a great resource for students last year and YS Schools are excited to be able to provide free meals to all students again this year. Although meals will be free again for all students, YS Schools are urging all families who have had any economic changes over the past year to complete an application for free or reduced meals. These applica- tions are important because they are used as an indicator in the fee waiver process and students who qualify for these services may also receive waived fees for instruc- tional materials. These applications also have larger implications for Yellow Springs Schools. The percentage of free and reduced appli- cations has a direct correlation with the federal dollars the schools receive, which allows YS Schools to provide a variety of academic supports for students. Please help YS Schools support stu- dents by completing an application.
