Request For Proposals: Area Agency On Aging, PSA 2
The Area Agency on Aging, PSA 2 seeks Letters of Intent from ser vice providers who are interested in receiving a three year contract for fiscal year January 1, 2016 – December 31, 2018 through a competitive Request For Proposal process for provision of specified ser vices for older adults age 60 and older, residing in Champaign, Clark, Darke, Greene, Logan, Miami, Montgomer y, Preble and Shelby counties under the following programs:
• Older Americans Act Title III Community Based Services
• Care Coordination
• Caregiver Support – for older adults who have an identified caregiver who is involved in their care.
• Home Repair Services – targeted for older adults age 60 and older in economic and social need.
